We will be happy to mail your patient registration forms to your residence, or you can download the forms here. Complete them at your convenience, and bring them to your first visit for prompt registration. If you have questions, simply call our office.

Dental Insurance

We know that you work hard for your dental benefits, so we’ll be happy to file your claim and follow up with your insurance company. In most cases, patients pay a copay at their visits, and the insurance company reimburses our office for the difference. This is called accepting assignment of benefits.

We can estimate your benefits and provide you with an approximate cost for your portion of the visit or treatment plan. If for any reason your insurance company does not pay their portion, the balance will become your responsibility. Rest assured, we will explain your insurance coverage and treatment plan costs before providing treatment, so you will understand your financial obligation. If you have any questions, please ask one of our front desk associates.
